• Requested by colleges/universities and boarding schools abroad. 
• The Requesting institutions usually have their form to be filled out.
• The form has sections to be filled by the applicant and sections to be filled by the health care provider.
• The medical usually has 3 main components which are indicated below

• Medical history and physical examination - main objective is to check for any chronic and communicable diseases, also to check for any allergies
• Immunization history/vaccination clearance – Completion and proof of childhood vaccination series being mandatory, The required and recommended vaccines of adults depend on the requesting institution's preference, usually required vaccines are MMR, Varicella and Tdap , Flue shot, adult polio, Hepatitis B, Meningococcal (ACYW and B), and among the Recommended include HPV, Hepatitis A, Typhoid, yellow fever...
• *Most schools require initial booster dose of tetanus vaccine to be Tdap that is given after the age of 7, and subsequent boosters that is given every 10 years to be either TD or Tdap.
• TB screening- Required for all applicants that reside in / is from WHO high TB prevalence countries, usually IGRA (TB blood test) is required – intended to check for the presence of latent TB if positive- Chest X-ray is required to rule out Active TB, if chest x-ray unremarkable then submitted and finished, *some institutions Require Treatment of latent TB before enrolment
• *Rarely some institutions may ask for certain lab works including serum Hemoglobin/CBC (to check for the presence of anemia- to determine fitness for sports activity)

Note: -

• The vaccinations we currently provide at 51±¬ÁÏÍøare Hepatitis B, MMR, TD, Tdap, MCV4, IPV, and Varicella all the other required vaccines mentioned above are usually not available in the country- in such instances we write them a medical certificate indicating the unavailability, which they submit along with the form, (Please be aware that the availability of this vaccines at our center is subject to change)
• TB  IGRA test is available at our center,    it is recommended to give blood sample for IGRA  test with in 24 hr of getting   live vaccines(MMR, Varicella
